New Member Registration Instructions
1. Please read the “Registration Cover Letter”.
2. Complete and sign the “Registration Form”, the “Parent/Guardian Code of Conduct” and the “Waiver of Liability” form and mail to“KIDS, Inc, PO Box 1397, Lowell, MA 01853” along with the registration fee of $25.
( $25 fee is per registrant )
The mail is picked up once a week and you will be contacted by Administration when the forms are received.
PLEASE NOTE: Registration forms will not be accepted without the fee and a signed copy of the Parent/Guardian Code of Conduct.
Group Home Member Registration Instructions
If you are a member of a Group Home please complete the “Group Home Registration Form”, the
“Parent/Guardian Code of Conduct” and the “Waiver of Liability” form and mail to
“KIDS, Inc, PO Box 1397, Lowell, MA 01853” along with the registration fee of $25.
( $25 fee is per registrant)

To Volunteer
Please complete the “Volunteer Application” and mail to the KIDS address on the form. You will be contacted by our Volunteer Coordinator once the forms have been received and processed. This can take up to a few weeks.
